Consultation Forms

Please create your own version of a medical history form & Aftercare Instruction (see below).  It's important that you keep a record of these to keep your insurance valid, as if you do not discover of a client's medical history you may be liable if the client has a reaction to a treatment, and you didn't find out about it first. 

Make sure you get them to sign the form as acceptance of risk and agreement to the procedure.
